[Platform Announcement] Deworming Record Template (2026 Version): Write according to this structure to avoid confusion during subsequent reviews.

Many users diligently deworm their pets, but two common problems arise when reviewing their records: First, they record the date but don't know which product and dosage it corresponds to; second, after a mild reaction, they can't find any contextual information. This makes "having records" become "difficult to review." This announcement provides a readily applicable deworming record template. The goal isn't to write professionally, but to allow for quick answers during any future review: what was used, when was it used, the pet's condition at the time, the subsequent reaction, and how to proceed next time.

It is recommended that each deworming record include at least the following fields: pet name, type of deworming (internal/external/combined), product name, batch or specification, dosage, method of administration, time of administration, pre-administration condition, observation results 24-72 hours after administration, any abnormalities and corresponding actions, and planned next time. You don't necessarily need to write a long record every time, but the fields should be as complete as possible to avoid misjudgments due to missing information when the season changes or products are switched.

In the "pre-administration condition" section, it is recommended to record three basic observations: appetite, mental state, and bowel movements. Many people overlook this step, making it difficult to determine whether subsequent changes are a reaction to deworming, a continuation of the original condition, or influenced by other factors. When observing after medication, it's recommended to at least record "any vomiting/diarrhea, local skin reactions, changes in mental state, or changes in appetite," and specify the duration. If there are no abnormalities, writing "no obvious abnormalities" is also valuable information.

Dosage-related suggestions should be as complete as possible. For example, "Based on current weight X kg, using Y specification, actual dosage Z." If the weight has changed significantly compared to the previous dose, it's best to add a sentence like "This dose was adjusted based on the current weight." This is crucial for subsequent judgment. If you have multiple pets, avoid mixing up information; include the pet's name and deworming type in the title, such as "Tuanzi | External Deworming | Drops | Routine."

For "Explanation and Handling of Abnormalities," it's recommended to only describe the facts and actions, not arbitrary conclusions. For example, "Slight soft stool appeared 6 hours after administration, lasting approximately 12 hours; mental state and water intake were normal during this period, and the patient recovered the next day." This kind of description is more valuable for review than "The medication was probably unsuitable." If similar phenomena occur again, you can more quickly determine whether it's an isolated incident or a recurring pattern.

When planning your next appointment, don't just write "next month." It's best to specify the exact date or time range and add a triggering condition: "Does it need to be brought forward/postponed?" For example, "If bathing frequency or outdoor grassy activities increase recently, we need to assess whether to adjust the schedule." This way, even when you're busy, you can quickly know what to do next.

Finally, let's emphasize the boundaries: Platform records are for routine health management and do not replace veterinary diagnosis. If you experience persistent vomiting, significant lethargy, abnormal breathing, persistent diarrhea, or other significant discomfort after deworming, please seek medical attention immediately. We will also be releasing a series of content such as "Immunization Record Templates" and "Medical Visit Review Templates" to help you turn each routine care into a long-term usable data asset.
